【Report (What we have done)】

Our committee (Resident-Fellow Committee; RFC) held a seminar called “The 5^th RFC Seminar featuring cardiovascular medicine in November 2019. Holding “RFC Seminars” is one of our main projects to broaden the community of RFC in Japan. In November, more than 40 Japanese junior residents and fellows gathered in Tokyo to learn together to strive for excellence in internal medicine frontlines.

Also, we were to help organize the Joint Session of the Japanese Society of Internal Medicine (JSIM) and the American College of Physician Japan Chapter (ACPJC) in the margin of the 2020 JSIM Annual Meeting in April. Besides, we had planned six different seminars and workshops for the 2020 American College of Physician Japan Chapter. Due to the COVID-19 pandemic, both of them were unfortunately canceled.

【Plan (What we are going to do)】

We are planning to organize a “Virtual RFC Seminar“ in collaboration with US-trained attendings at the beginning of the FY2020. Amid the era of social distancing with the COVID-19 outbreak, we believe our committee may work as a “communication and collaboration hub” to connect exceptional Japanese residents and fellows. Also, we will work with the Council of Resident/Fellow Members of ACP to share the best practices to enhance the quality of postgraduate internal medicine training while fighting against the COVID-19 outbreak.

The 5th RFC seminar – Deep Dive into Cardiology –

I believe every one of you might have felt cardiology is difficult to learn. In the 5^th Resident-Fellow Committee (RFC) Seminar on November 10, is a solution for those people.

The RFC Seminar is held up to twice a year. In the seminars, we invite internal medicine subspecialists as well as doing a brief “Dr’s Dilemma” like session to disseminate the high-tiered, essential internal medicine skills to medical students and residents. Through the seminars, we aim to get them together to strengthen the community of ACP as well. The 5^th RFC Seminar was held in Nihonbashi, Tokyo. The guest lecturers were Dr. Akio Kawamura, Professor and Director of Department of Cardiology, International University of Health and Welfare, and Dr. Atsushi Mizuno from Cardiovascular Center of St. Luke’s International Hospital. Approximately 30 participants gathered Tokyo for the seminar.

In the lecture of Dr. Kawamura, he showed us his career path as a cardiologist as well as his view on medical education and treatment of patent foramen ovale (PFO). He also briefly summarized the clinical implication and rationale to treat PFO, which was meaningful to generalists as well.

Dr. Mizuno organized a session in a group-work style to let participants learn “how to manage heart failure and its complications”. Through the session, participants acquired skills of initial managements, differential diagnosis and clinical management. All the participants apparently enjoyed his session.

Lastly in the afternoon, the RFC committee presented a small Dr’s Dilemma-like session. Not only the participants solved the quizzes, Dr. Mizuno also added clinical implications and reality in the frontline clinical field to every question and answer. We are sure that this was the way to get the most from clinical quizzes.

RFC plan to continue seminars like this. Through the seminars, annual meetings and so on, we hope to contribute to the further development of ACP Japan Chapter as well as the field of Internal Medicine in Japan.

ACP日本支部 Resident-Fellow Committee主催　「第5回RFCセミナー」

「循環器って難しい」、そんなことを一度は思ったことはありませんか？今回のResident-Fellow Committee主催「第5回RFCセミナー」は、そのような若手医師にありがちな苦手意識を忘れさせるような、濃密な企画となりましたのでご報告します。

RFCセミナーは、年に2回程度、内科各分野のスペシャリストを講師としてお招きし、特別講演やMKSAPの類題などを通じて、やる気溢れる学生や研修医をさらにワンステップ進化させ、ACP日本支部の輪を若手の中で広げることを目的としたものです。
今回の舞台は11月10日（日）、東京都日本橋。「循環器」をテーマに、河村朗夫先生（国際医療福祉大学医学部循環器内科主任教授）、水野篤先生（聖路加国際病院心血管センター医幹）をお招きし、全国各地から学生、初期・後期研修医、さらにはベテラン医師まで、約30名が集いました。

河村先生の講演では、先生が循環器内科医として歩まれてきた経歴のご紹介から、卵円孔開存の病態と治療、医学教育者としての在り方、考え方についてなど、幅広くお話頂きました。会場が笑いに包まれるような雰囲気の中でも、卵円孔開存の先進治療や臨床的意義等、驚かされる医学的知識もちりばめられた講義で、ついつい聞き入ってしまいました。

水野先生の「合併症を考慮した心不全治療」をテーマとした症例検討会では、心不全診療の初期対応、鑑別診断、そしてマネジメントまで、全てを網羅的に、グループワークを通じて学びました。循環器分野に限らず、医師としての学習、後輩指導のテクニック等もお話頂き、会場全体が盛り上がっていたのが印象的でした。

そして、午後はRFC委員が作成したMKSAPの類題を用いた「診断道場」でした。このセッションでは、クイズ形式で臨床問題を参加者全員で解いていくだけでは無く、水野先生から日本での実臨床や、ご自身の経験を踏まえたコメントを問題毎に頂き、机上の空論に留まらない実践的な知を習得することができました。

セミナー全体を通して、臨床現場の第一線で活躍されている専門医の先生方から各々の専門領域やジェネラリストへの示唆について聞くことができ、参加者は皆循環器診療におけるリアルな臨床現場を垣間見ることができたと思います。RFCは今後もこのような企画の立案等を通じて、ACP日本支部、日本の内科診療の発展に貢献していきたいと思っています。

ACP Japan Chapter Resident Fellow Committee Interim Report

当委員会（以降RFC）では、2018年度年次総会にて実施した以下の企画（①RFC企画として新専門医制度に関するシンポジウム ②招聘講師による内科医師のための救急外来マネジメントワークショップ ③若手医師のための懇親会）、及び、10月に開催した第4回RFCセミナーについてご報告させていただきます。

年次総会で実施した①については、主に総合内科若しくは総合診療科で活躍されている先生方からのインプットをいただき、始動したばかりの専門医制度に対して現場の懸念や問題点を明確化する良い機会となりました。同制度は現在も様々な懸念事項を含みつつも継続されており、2019年度も継続して議論する意義があると考えています。本年度の反省を活かし、スペシャリスト、新専門医制度にて実際に研修している医師からのインプットを入れ込みつつ、より良い制度を作っていくために現場から何らかの提言が出来れば、と考えています。②及び③では、それぞれ50名、60名程度の参加者が相互対話的にディスカッション、懇親を図ることができ、参加者にとって実りある時間を提供できただけでなく、日本でACPの輪を拡げることにも貢献できたと考えています。

第4回RFCセミナーでは、7割の非会員を含む40名の参加者が集い、「ジェネラリストのためのリウマチ膠原病」のテーマで共に学ぶ機会を提供しました。募集開始後2週間以内に定員を超える参加希望者が充足され、RFCに限らず、ACP日本支部の若手医師間における知名度向上、充実した内容の企画を行うキャパシティのある団体としてのプレセンスのアピールを行いました。また、MKSAPを題材とした自己学習方法の提案及びセミナーの需要が非常に高いことを確認でき、今後も年2回のRFCセミナー及び総会企画の立案・実施を継続して、MKSAPを前面に出してACPの日本におけるプレセンス向上にも寄与していくことを計画しています。今後とも皆様のご支援の程をよろしくお願い申し上げます。

I departed from Japan to New Orleans with determination to fight against the United States teams in the Dr’s Dilemma National Competition in ACP Internal Medicine Meeting 2018. Dr’s Dilemma is a competition to test the medical knowledge of around 50 teams, each representing each ACP chapters around the World. I participated in the competition with doctors from Nerima Hikarigaoka Hospital, the champion team in the 2017 ACP Japan Chapter Dr’s Dilemma Competition. In the elimination round, we fought against 4 teams including the state of Washington and Massachusetts. Quizzes were first read loud by the speaker, and the scripts were shown up on the screen afterward. Although we could reach the final question, we could not go through to the semi-final round. We ended up being the 4^th, taking out the Massachusetts team. Not only I could compete in the great competition, the conference itself also gave me opportunities to expand the wave of exchange with physicians around the world, making the experience was an one chance in a million, I deeply appreciate everyone in the ACP Japan Chapter and Okayama University Hospital, Department of General Medicine, for substantial support. Finally, I would like to thank ACP Japan Chapter for supporting us with travel grants.

「英語が母国語のチームに如何に立ち向かうか」、少しでも良い結果を残すべく2018年度ACP 2018年度総会に向けて出国しました。Dr’s DilemmaはACP Chapterを有する州、地域、諸国から約50の代表チームが集い、内科の知識を競う、いわばチーム対抗全米クイズ大会です。クイズ大会というと陳腐に聞こえるかもしれませんが、Dr’s Dilemmaに向けて丸一年かけて準備している州もあるほど、価値のある大会です。今回、2017年ACP日本支部総会で開催されたDr’s Dilemmaで準優勝となり、優勝チームメンバーの練馬光が丘病院のお二人とタッグを組み、日本代表としてこの栄誉ある大会に参加しました。過去に日本チームは予選突破を果たしたことがなく、必然的にそれが大目標となりました。約50チームある中で、予選通過できるのは上位20チームと狭き門ですが、全力で挑みます。

予選は5チーム程度の小グループに分けて開催され、各小グループの上位2チームが準決勝に進むことが出来ます。我々のチームには強豪Washington州、Massachusetts州がおり、上位チーム常連組との勝負に不安を抱えつつも思い切って突撃。クイズのスタイルは一問一答方式で、まずスクリプトが読み上げられた後に文章が表示されるため、リスニング能力と反射神経が要求されます。日本チームはなんとかプラスの点数（問題を誤答すると点数が引かれる）で最終問題に挑みましたが、予選4位で準決勝進出ならず。Massachusetts州を上回ったのは収穫でしたが、来年以降にもこの経験を生かしていこうと思います。また、総会自体は非常に大規模で、日本を代表する世界レベルの先生方との交流も図ることができ、まさに千載一遇のチャンスをつかんだという思いでした。最後になりましたが、今回Dr’s Dilemmaに出場するにあたりACP日本支部から旅費及び参加費の一部をサポートいただきましたことに御礼申し上げると共に、温かい応援をしてくださったACP日本支部、自施設の皆様に感謝申し上げます。

The 3rd Resident-Fellow Committee Seminar “Setting Out for the World Ahead!”

　We are glad to report that we have successfully finished the 3^rd RFC seminar with a slogan “Setting Out for the World Ahead!” on April 15, 2018. In the seminar, we set up 2 venues: Room A for ones who want to acquire the most important skills related to internal medicine in daily clinical encounters, and Room B to for young doctors interested in the evidence-based medical educational skills. Despite April, the busy first month of the fiscal year, there were 62 participants total. In the room A, we have invited 2 significant guest speakers; Dr. Yuka Kitano from the St. Marianna University School of Medicine, Yokohama City Seibu Hospital, and Dr. Sou Sakamoto from the Juntendo University, Nerima Hospital. In a 90-minute session each, Dr. Kitano held the session “Practical 5-min Teaching Skills for Clinicians”, for which she even has a blog. Dr. Sakamoto gave an interactive lecture “Approach for Patients with Altered Mental Status”. All the 43 participants in the Room A seemed to be immersed in the world of those leaders, with serious yet amused looks on their faces. Dr. Tadayuki Hashimoto from Hashimoto City Hospital gave us a workshop with a motto “Residents as Teachers”. It was done in a completely bidirectional style with a lot of group works. Almost all the participants gave back us good feedbacks after all. In the 63 participants, 40 of them were ACP non-members, and 7 of whom enrolled onsite! We are looking forward to continuing the seminar, anticipating broader yet more solid committee management.

第3回RFCセミナー盛況にて終了！

「あなたも世界の内科医に」を標語に、若手医師をターゲットに開催しているRFCセミナー、第3回を2018年4月15日に開催しました。今回は即戦力となる内科知識のブラッシュアップを目指すA会場、医学教育力向上を目指すB会場の二本立ての企画です。年度初めにも関わらずA会場 43名 / B会場 19名の計62名の参加登録がありました。A会場ではDr’s Dilemma形式の症例クイズを通じて臨床力を鍛えるRFC企画「Visual Diagnosis」、聖マリアンナ医科大学横浜市西部病院北野夕佳先生による「実践！5分間ティーチング」、順天堂大学医学部附属練馬病院坂本壮先生による「意識障害へのアプローチ」の三本立て。B会場では橋本忠幸先生を中心に「Residents as Teachers」をテーマに双方向性ワークショップを開催しました。参加者からのフィードバックも良好で、非会員40名程度、当日の会員登録が7名と、新規会員のリクルートという意味でも大成功だったと思います。今後も第4回、第5回と、継続運営及び発展を目指し、上月委員長を中心にRFC一同頑張って参ります。
