REPORT: What we have done

Even now, it is still in the corona-era, and face-to-face meetings have not been held since last December. Furthermore, there is a history that ACPJC 2020 was not held this year unfortunately, and no project has been completed since the committee was reorganized last July. We are currently continuing discussions on the mailing list for the topic mentioned in the PLAN below.

PLAN: What we are going to do

Evaluation and consideration of the integrity of online medical care/telemedicine

In Japan, which has a super-aging society, online medical care/telemedicine has been in the limelight since 1997 in order to solve the uneven distribution of doctors in metropolitan areas and the impaired accessibility to HCFs in rural areas. Since then, in 2018, concerning social issues such as rapidly expanding aging society across Japan, IT progression in this field was accelerated aiming to reduce outpatient visits as a patient factor, and to popularize telemedicine as part of reforming the working style of doctors (unfortunately, which was not noticeably spread at that time).

As the new coronavirus infection (COVID-19), which began in the winter of 2019, is spreading again, the government has been taking the initiative in accelerating to spread of telemedicine using phones and/or computer networks as solution tools for the concerning situation (e.g. getting become difficult for patients to consult doctors directly). Then, this type of medical care is once again in the limelight, and a scheme for its proper operation having been formed now. In response to these global and social movements, we all HPPC members are continuing discussions on the mailing list focusing on this topic.

Generally, telemedicine is conducted between doctors and patients, and it is possible to examine, monitor using some extra-equipment, and explain the diagnostic results of patients not in urgency, but in principle, the first examination must be face-to-face, and prescription can be made after the examination. However, the technology in this field is progressed largely from the IT professional’s view, not from the clinician’s view. Therefore, from the clinician’s standpoint, we are currently discussing the limitation of telemedicine, the practice of EBM, CW/HVC, and the method of establishing professionalism within “Human to Human (H2H)” communications, and we would like to make recommendations from this committee in the future.

Report: What We Have done

We planned “The Road to FACP – Why do we aim for it?” and held at the 2019 ACP Japan Chapter Annual Meeting.

Plan: Where we are going to go

1) Planned and going to have a workshop at the 2020 chapter meeting titled “The road to an FACP – Why do we aim for it? -“

2) Continue to develop a model to resolve common challenges on medical professionalism

3) Preparation of criteria for COI disclosure of roles at ACP Japan Chapter (ACP-JC) and COI disclosure at annual meeting of ACP-JC

4) Attempt to update the page of professionalism on the homepage of ACP-JC

5) Attempt to publish Choosing Wisely lists as the ACP-JC based on questionnaire of ACP-JC members

6) Examine and discuss “End of life care” that would more suitable for Japanese medical circumstances

7) Analyze the attitude of ACP headquarters towards COI and report it to the ACP-JC members

The road to an FACP – Why do we aim for it? –

On the second day of ACP Japan Branch Annual Meeting 2019, the afternoon of June 9, we held a session with the above title as a project of HPPC. There are 337 FACPs, 6 MACPs and 1 Honorary Fellow as of December 18, 2018 in the ACP Japan branch. By listening to the reason why they aimed at Fellow, how they changed their daily work by becoming Fellow, and how they would like to act as Fellow in the future, we hope more member aim to FACP.

We had conducted a similar session at the annual meeting last year, and in this project, we also conducted a preliminary questionnaire on acp-exchange etc. We ask FACP and MACP about the process of becoming FACP/MACP and the change after that, etc., we ask Members about the image of FACP and whether or not they are aiming, and how they think they would change by becoming a FACP.

In the session, after explaining the purpose first, we presented the results of the preliminary questionnaire. Based on the results, valuable comments were received from Dr. Kenji Maeda, the current Governor of the Japan Chapter, Dr. Fumiaki Ueno, the former Governor of the Japan Chapter, and Dr. Noriko Yamamoto, chair of the Women’s Committee. As a result, a more meaningful message was delivered to everyone who participated, as well as to HPPC members. We would like to deeply thanks to the three commentators for taking part in this session despite the busy period. There are various motives when becoming a Member or Fellow, but being a Fellow gives them a mental motivation such as pride, confidence, and a sense of responsibility that seems important to continue the profession of a doctor.

After that, participants, commentators, and HPPC members joined to form three small groups, and group discussion were conducted along the theme. Participants exchanged their opinions easily, and active conversations were held in all groups. Finally, we made a summary and ended this session.

Members who participated in the session could hear the story from Fellow in the group discussion, so they would have thought of becoming a Fellow. Thank you very much for all the participants who have been listening diligently. It is hoped that more people aim for Fellow with this project as opportunity, but eventually it is the hope of us that ACP Japan chapter members will be more by repeated such projects.

ACP日本支部年次総会2019の2日目,6月9日の午後,HPPC企画として上記タイトルでセッションを開催した.ACP日本支部には2018年12月18日現在で337名のFACP,6名のMACP,1名のHonorary Fellowがおられるが,その方々はどのような思いでFellowを目指されたのか,Fellowになることでどのような違いが日常業務に生まれたのか,今後どのように活動していきたいか,といったことを聴くことで,より多くの方がFellowを目指しFACPになってもらいたいという意図があったからである.

同様のセッションを昨年度の年次総会でも行っているが,今回の企画でも,acp-exchangeなどで事前アンケートを行い,FellowやMasterの方々にはFellowになった経緯や昇格後の変化などについて,Memberの方々にはFellowのイメージや目指しているかどうか,Fellowになることで自身がどのように変化すると思うか,などについて回答を募っていた.

セッションでは,まず趣旨説明のあと,事前アンケートの集計結果をセッション中に発表した.結果をもとに,コメンテーターとして御登壇いただいた前田賢司・現支部長と上野文昭・前支部長,Women’s Committeeの山本典子委員長のお三方から貴重なコメントを頂戴した.これにより参加された皆さんにもHPPC委員にもより意義深いメッセージが伝えられることになった(3人の先生には会期中ご多忙にも関わらず本セッションにご登壇いただきありがとうございました.心より感謝申し上げます).MemberやFellowになった時の動機は様々であるが,Fellowになることで誇りや自信,責任感など,医師という職業を継続するうえで重要だと思われる精神的な動機付けが付与されることが多い,ということが昨年同様に認識される結果であった.

このあと,参加者とコメンテーターの諸先生,HPPC委員が混ざり,スモールグループを作ってテーマに沿ったグループディスカッションを行った.広い会場では質問しにくい事も気楽に意見交換できる環境だったため,どのグループでも盛んな会話が交わされていた.最後にセッションのまとめを行って本セッションを終了した.特に後半のグループディスカッションでFellowである日本支部の同僚からいろいろ話を聞くことができたことで,参加したMemberの皆さんにとってもFellow取得に気持ちが傾いてくれたのではないかと期待している.熱心に聴講および意見交換してくださっていた参加者の皆さん,本当にありがとうございました.

本企画を契機にFellowを目指す方がより多くなることを願っているが,ひいてはこのような企画が繰り返されることによってACP日本支部会員がより多くなり,ACP日本支部がさらに発展することが委員一同の望みでもある.

The road to an FACP – Why and how do we aim for it? –

 On the second day of ACP Japan Chapter Annual Meeting 2018, the morning of June 3, we held a session with the above title as a joint project between HPPC and CMC. Although there are 320 FACPs and 6 MACPs as of July 1, 2017 in the ACP Japan Chapter, it was because we had the intention to have more member become FACP by listening to answers to questions “ Why did they aim for Fellow? ”, “ What kind of difference can be seen in daily work by becoming Fellow? “, and “How do they want to act in the future? “

 At the time of planning and submission, CMC was also planning a session to announce the procedure of Fellow application. Since the planning direction is the same, we decided to collaborate.

In the session, after opening remarks, we project the Convocation video which was held at ACP annual meeting (Internal Medicine). Prior to this project, we conducted a preliminary questionnaire using acp-exchange mailing list etc. Fellows and Masters were asked to answer in a questionnaire about the aim for Fellow and the change after becoming Fellow, and members were asked to answer in a questionnaire about the image of Fellow, whether they are aiming for Fellow, and whether they think that they would change themselves by becoming Fellow. So, after Convocation video presentation, we announced the results of the questionnaire and conducted a discussion based on the answer contents.

This time, Dr. Fumiaki Ueno (Governor, ACP Japan Chapter) and Dr. Kenji Maeda (Governor-Elect, ACP Japan Chapter) fortunately participated as a commentator. For that reason, clear focus was placed on this project, and a more meaningful message was conveyed to both the participants and the committee members as well (Dr. Ueno and Dr. Maeda, we thank you for taking the time to attend this session despite being busy during annual meeting). Although the motivation when respondents became Member or Fellow was various, we got impression that spiritual incentives such as pride, self-confidence, sense of responsibility, and so on, are given mental motivation to continue the occupation of a doctor by becoming Fellow.

After the discussion, there was a comment on the steps to apply for Fellow, and the session was summarized. Since the application procedure was explained in an easy-to-understand manner, for the participants, the method of acquiring Fellow became even clearer. Thank you very much for all the participants who had listened diligently.

We hope that more people aim for Fellow with this project as opportunity, but eventually, the two committees hope that number of members of ACP Japan Chapter will increase by repeating such a project.

「FACPへの道 - なぜ、そしてどのようにそれを目指すのか- 」

ACP日本支部年次総会2018の2日目、6月3日の午前に、HPPCとCMCの共同企画として上記タイトルでセッションを開催した。ACP日本支部には2017年7月1日現在で320名のFACP,6名のMACPがおられるが、その方々はどのような思いでFellowを目指されたのか、Fellowになることでどのような違いが日常業務に生まれたのか、今後どのように活動していきたいか、といったことを聴くことで、より多くの方がFellowを目指しFACPになってもらいたいという意図があったからである。企画提出時に、Fellow申請の手順などをアナウンスすべくセッションを企画されていたCMCの皆さんと、企画の方角が一緒なので一緒にやりましょうということで共同企画にさせていただくことになった。

セッションでは、まず趣旨説明のあと、米国内科学会Internal Medicineで行われるConvocationのビデオ映像を供覧した。今回の企画にあたり、acp-exchangeなどで事前アンケートを行い、FellowやMasterの方々にはFellowになった経緯や昇格後の変化などについて、Memberの方々にはFellowのイメージや目指しているかどうか、Fellowになることで自身がどのように変化すると思うか、などについて回答を募っていたので、Convocationのビデオ供覧に次いで、その結果をセッション中に発表して、回答内容をもとにディスカッションを行った。今回、コメンテーターとして上野文昭・現支部長と前田賢司・次期支部長のお二人にご登壇いただけたことで、本企画に一本芯がしっかり入り、参加された皆さんにも両委員会委員にもより意義深いメッセージが伝えられることになった(両先生には会期中ご多忙にも関わらず本セッションにご登壇いただきありがとうございました.心より感謝申し上げます)。MemberやFellowになった時の動機は様々であったが、Fellowになることで誇りや自信、責任感など、医師という職業を継続するうえでの精神的な動機付けが付与されることが多いような印象であった。

これらの情報交換のあと、Fellowを申請するためのステップについて解説があり、セッションのまとめを行って終了となった。申請ステップの解説により、参加された方々にとってはFellow取得への道程がより明解になったことと思われる。熱心に聴講してくださっていた参加者の皆さん、本当にありがとうございました。
