Thanks to present and past Governors of the ACP Japan Chapter and Chair of the Credentials/Membership Committee, I could be a new FACP at the ACP Internal Medicine Meeting 2018 at New Orleans this April.
I am most honored to be a FACP because I used to point out the needs of broader coverage of patients and illnesses in the clinical practice, in contrast to the research activities. In my case, I am always trying to be a physician, and a generalist, rather than an internist, rather than a neurologist, rather than a critical care neurologist. Also, I am most pleased to be a FACP because I was involved in the project of the Japanese Society of Internal Medicine for the translation of the ABIM/ACP/EFIM Physician Charter and Medical Professionalism in 2002, and also in the project of creating the Physician Charter in Japan.
I would like to specific contribute to the ACP and ACP Japan Chapter as a FACP and a FAAN (Fellow of the American Academy of Neurology).

このたび、米国内科学会年次総会でFACP称号を授与戴きました。ご尽力戴きました上野文昭支部長、前田賢司次期支部長、小林祥泰前支部長、新谷英滋委員長ほかの先生方に深く感謝申し上げます。
私はgeneralist、specialist、専門医、総合内科専門医をめぐる議論に、「研究は狭くても良いが、診療は臨床医として少しでも広くカバーできるべき」と強調して参りました（永山正雄. 医学のあゆみ, 2011）。私自身の場合で申しますと、脳卒中をはじめとする神経救急・集中治療の専門家である前に神経内科医であり、その前に内科医、臨床医、医師であるべきと考え、実践して参りました。2004年からは、日本内科学会認定内科専門医会プロフェッショナリズム委員会、大生定義委員長）委員として、ACP憲章（2002）邦訳、わが国独自の医師憲章を模索する活動に参加させて戴き、その成果を「米欧合同医師憲章と医のプロフェッショナリズム-日本版内科専門医憲章策定をめざすプロジェクトの成果」（永山正雄、前田賢司ほか、内科専門医会誌, 2016）、書籍「白衣のポケットの中」（医学書院, 2009）として発表する機会も戴きました。
すでに1998年に米国神経学会（AAN）フェロー（FAAN）の称号も授与されておりますので、順序が逆になってしまいましたが、20年振りに訪れた米国New Orleansの地で、荘厳なConvocation Ceremonyに参加し、世界の同学の士とともにFACP称号を授与戴きました。大変感動しmotivationが高まる一方、欧米は勿論、とくに新興国からの新FACPの著しい台頭に鑑み、危機感も覚えました。彼らの多くはfriendlyですが、その向上心、hungry精神は高いことを、留学者数、論文数、大学等の国際ランキング等において埋没しつつあるわが国に伝え、hungry精神を育むべく後進を刺激することの重要性、FACPはゴールでは無いことを再認識して戴く教育の重要性、ACP日本支部のプレゼンスをさらに高めるための実行力の必要性を強調し、実践して参りたいと思います。
The road to an FACP – Why and how do we aim for it? –

On the second day of ACP Japan Chapter Annual Meeting 2018, the morning of June 3, we held a session with the above title as a joint project between HPPC and CMC. Although there are 320 FACPs and 6 MACPs as of July 1, 2017 in the ACP Japan Chapter, it was because we had the intention to have more member become FACP by listening to answers to questions “ Why did they aim for Fellow? ”, “ What kind of difference can be seen in daily work by becoming Fellow? “, and “How do they want to act in the future? “
At the time of planning and submission, CMC was also planning a session to announce the procedure of Fellow application. Since the planning direction is the same, we decided to collaborate.
In the session, after opening remarks, we project the Convocation video which was held at ACP annual meeting (Internal Medicine). Prior to this project, we conducted a preliminary questionnaire using acp-exchange mailing list etc. Fellows and Masters were asked to answer in a questionnaire about the aim for Fellow and the change after becoming Fellow, and members were asked to answer in a questionnaire about the image of Fellow, whether they are aiming for Fellow, and whether they think that they would change themselves by becoming Fellow. So, after Convocation video presentation, we announced the results of the questionnaire and conducted a discussion based on the answer contents.
This time, Dr. Fumiaki Ueno (Governor, ACP Japan Chapter) and Dr. Kenji Maeda (Governor-Elect, ACP Japan Chapter) fortunately participated as a commentator. For that reason, clear focus was placed on this project, and a more meaningful message was conveyed to both the participants and the committee members as well (Dr. Ueno and Dr. Maeda, we thank you for taking the time to attend this session despite being busy during annual meeting). Although the motivation when respondents became Member or Fellow was various, we got impression that spiritual incentives such as pride, self-confidence, sense of responsibility, and so on, are given mental motivation to continue the occupation of a doctor by becoming Fellow.
After the discussion, there was a comment on the steps to apply for Fellow, and the session was summarized. Since the application procedure was explained in an easy-to-understand manner, for the participants, the method of acquiring Fellow became even clearer. Thank you very much for all the participants who had listened diligently.
We hope that more people aim for Fellow with this project as opportunity, but eventually, the two committees hope that number of members of ACP Japan Chapter will increase by repeating such a project.

「FACPへの道 - なぜ、そしてどのようにそれを目指すのか- 」

ACP日本支部年次総会2018の2日目、6月3日の午前に、HPPCとCMCの共同企画として上記タイトルでセッションを開催した。ACP日本支部には2017年7月1日現在で320名のFACP，6名のMACPがおられるが、その方々はどのような思いでFellowを目指されたのか、Fellowになることでどのような違いが日常業務に生まれたのか、今後どのように活動していきたいか、といったことを聴くことで、より多くの方がFellowを目指しFACPになってもらいたいという意図があったからである。企画提出時に、Fellow申請の手順などをアナウンスすべくセッションを企画されていたCMCの皆さんと、企画の方角が一緒なので一緒にやりましょうということで共同企画にさせていただくことになった。
セッションでは、まず趣旨説明のあと、米国内科学会Internal Medicineで行われるConvocationのビデオ映像を供覧した。今回の企画にあたり、acp-exchangeなどで事前アンケートを行い、FellowやMasterの方々にはFellowになった経緯や昇格後の変化などについて、Memberの方々にはFellowのイメージや目指しているかどうか、Fellowになることで自身がどのように変化すると思うか、などについて回答を募っていたので、Convocationのビデオ供覧に次いで、その結果をセッション中に発表して、回答内容をもとにディスカッションを行った。今回、コメンテーターとして上野文昭・現支部長と前田賢司・次期支部長のお二人にご登壇いただけたことで、本企画に一本芯がしっかり入り、参加された皆さんにも両委員会委員にもより意義深いメッセージが伝えられることになった（両先生には会期中ご多忙にも関わらず本セッションにご登壇いただきありがとうございました．心より感謝申し上げます）。MemberやFellowになった時の動機は様々であったが、Fellowになることで誇りや自信、責任感など、医師という職業を継続するうえでの精神的な動機付けが付与されることが多いような印象であった。
これらの情報交換のあと、Fellowを申請するためのステップについて解説があり、セッションのまとめを行って終了となった。申請ステップの解説により、参加された方々にとってはFellow取得への道程がより明解になったことと思われる。熱心に聴講してくださっていた参加者の皆さん、本当にありがとうございました。
本企画を契機にFellowを目指す方がより多くなることを願っているが、ひいてはこのような企画が繰り返されることによってACP日本支部会員がより多くなることが両委員会一同の望みでもある。

自身にとっての原点を維持すること 研修医時代の恩師とともに

ACP Japan chapterの皆様はじめまして。この度FACPに昇格いたしました、徳島大学腎臓内科の岸 誠司と申します。今回のFACP昇格にあたり、寄稿の機会をいただきました。誠に光栄なことであり、さらには舞鶴市民病院時代に医師としての基礎を身につけさせていただいた松村理司先生のHonorary FACP昇格と同時期ということも重なり（写真1）、今回この場を借りてFACP取得までの道のりを振り返らせていただきたいと思います。
私は徳島の田舎の出身で（やまびこ打線で有名になった池田高校のある、現徳島県三好市出身です。）。医師というとなんでも診てくれる内科の先生が自分の抱いている医師像でした。大学卒業後、何も迷うことなく内科医になることを選択し、日本の臨床研修の原点ともいうべき市立舞鶴市民病院でみっちりと内科医としての基礎をたたきこんでいただきました。医師にも働き方改革の波が押し寄せる昨今ではありますが、当時は月曜早朝から日曜の深夜まで、自己の医師としての能力を高めることに全力を注ぐ若い医師の集まりがそこにはありました。日々の回診、夕方のフィードバック、Common diseaseからrare diseaseまで病歴・身体所見からせまる臨床診断、臓器のみならず原因菌をも同定したうえで進める感染症治療、幅広い疾患をバックアップの元経験できる救急医療、教育にたけた米国人医師によるカンファレンスなど、現在でもどこを探してもないような理想の研修環境がそこにはありました。まさに“三つ子の魂100まで”の医師版“研修医時代の医師としての姿勢が医師人生を決める”であり、私の医師としての根幹はこの時期にしっかりと形成されたのは間違いありません。
舞鶴市民病院での研修の後、兵庫県立尼崎病院で腎臓内科医として研鑽を積みました。非常に症例の多い忙しい病院で、腎炎ネフローゼから緊急透析症例、さらには全身疾患の中の部分症としての急性腎障害など非常に多彩な症例を経験しました。また、ちょうどCKDや心腎連関などの概念が広がり始めた時期でもあり、伝統ある循環器内科のアクティビティが高いこともあって、腎臓内科医としていろいろな意味で鍛えられました。一方で腎臓内科医である前に総合内科医であるというプライドをもって働いておりました。当時は総合内科はなく、腎臓内科は全身を診る科という上司の理解もあり、“うちじゃないといわない”という精神で多くの症例に関わらせていただきました。これだけにとどまらず、舞鶴魂で積極性を発揮し、舞鶴市民病院で学んだ腹部エコーを継続する機会も頂き、もう一歩進んで消化管内視鏡の研修もさせていただくことが出来ました。
ちょうど現在の臨床研修制度が始まった時期でもあり、研修医教育にも積極的に関与することができました。先述のとおり、当時は尼崎病院には総合内科はなく、腎臓内科に入院する症例をみながら舞鶴で教わったこと、身に着けたことを後輩医師に伝えるべく努力しました。それは初代最優秀専攻医（研修医の投票で選ばれる指導医の表彰です）選出として評価を頂く機会につながり、とてもよい自信になりました。
尼崎病院の総合内科の立ち上げの際にも少しだけ関わることが出来ました。現在科長である吉永先生を藤本卓司先生、川島篤志先生のおられた市立堺病院にお連れし、総合内科を見学させていただいたこともよい思い出です。
総合内科専門医を尼崎病院在職中に取得しました。同僚の先生の情報でアメリカ内科学会に入会できるとのこと、舞鶴市民病院OBの血が騒ぎ、さっそくアメリカ内科学会に入会しました。
腎臓内科後期研修を終え、次のステップを考えました。病院を移り、さらなる研鑽を積むことも考えたのですが、自身の中に“米国留学”という次の目標が生まれました。とはいえ、USMLEを受ける、という選択肢には自分は至らず、徳島大学に帰り、学位を取って研究留学する、という目標を立てました。徳島大学に帰り、腎臓内科医員としての診療を開始し、併せて土井俊夫教授のもと基礎研究を始めました。地方国立大学は尼崎病院時代に始まった臨床研修制度の影響をもろに受けている感じでした。当時の医局の上司の先生に、FACPという制度があることを教えていただきました。その頃はとても遠い存在に感じており、FACPを目指す、ということは考えていませんでした。
学位取得後、Harvard Medical School/Brigham and Women’s Hospital, Renal Division, Bonventre Labに留学しました。文字通り、留学先を探すところからのゼロからのスタートでした。自身のCVと学位論文を持ち込み、直接交渉をしました。結果、なんとかボストンにたどり着き３年間ポスドクとして基礎研究を行いました。研究室での本業に加え、日常生活、ヒトとの出会い、すべてが刺激の連続でした。舞鶴時代に多くのアメリカ人医師と接していたことが自分の中で大きな自信でした。そして、今回のACP総会に留学時代のボスも演者として来られておりました。写真２
そして今回留学から帰国し、FACP申請、昇格に至りました。
以上振り返ってみますと、医師になってからのキャリアすべてが今回のFACP昇格につながっていたと考えられました。さらには、すべての過程において、舞鶴市民病院で身に着けた医師としての基礎があって、次のステップに進めたと確信できました。Diversityは世界を取り巻くkey wordでありますが, 私自身も上記のとおり、総合内科診療、臓器専門医、医学研究者としての様々なキャリアを積むことができました。これからもジェネラルとスペシャル、臨床医と医学研究者、の間を往復しながら進んでいきたいと考えております。今後も多くの内科医師の方がFACPを目指され、そして昇格されることを願って今回の体験記を終えさせていただきます。

I performed my presentation titled“Cavitary Lung Lesions in a Patient with Positive IGRA and PR3-ANCA are not Always due to TB or GPA: a Case Report of Right-Sided Infective Endocarditis”. It was an honor to receive first place in theKurokawa prize competition, the best abstract award (medical student section) at the annual conference of the ACP Japan Chapter 2018.
I am interested in clinical reasoning, so I often attend case conferences and journal club sat the General Internal Medicine department to read and discuss cases from theNew England Journal of Medicine. A doctor suggested that I write an abstract. However, I was a fourth-year student at that time and had not started my clinical clerkship yet, so I started asking about cases for the abstract. Since I have a passion for studying infectious diseases and rheumatology diseases, I still remember how excited I was when I took this case.
In this case, it was saddening to learn that is under standing of laboratory data led to them is diagnosis of the patient which resulted in anatrogenic exacerbation of the patient’s condition. We must consider bacteremia and infective endocarditis from the history and vital signs, and send a blood culture for examination. If the culture is positive, then we need to perform an echocardiogram. There is no doubt that his quality of life would have been improved with those few non-invasive procedures. A famous Japanese doctor who I respect says that the essence of the physician is the differential diagnosis. You can’t take note of the patient’s history, conduct a physical examination, perform a laboratory examination, and conduct the treatment without a differential diagnosis. So in this case, if infective endocarditis was included in the differential diagnosis in the early stages, we could have avoided such a situation by conducting a blood culture and an ultrasonography. It is said that a lot of unneeded laboratory examinations are performed in Japan. Some people even say “clicking the order button is not the prescription for a doctor’s anxiety to misdiagnosis”, but the lesson I learned from this case is that laboratory examinations sometimes startle us, not knowing the character of the laboratory examination.
In closing, I would like to take this opportunity to express my appreciation to you all. It would be impossible to have this award without your help. I will go forward on my newly set goal, which is to do a presentation at theACP Internal Medicine Meeting 2019 in Philadelphia, PA, USA. Thank you for your time.

ACP日本支部2018年次総会 黒川賞（若手医師部門）を受賞して

この度は、ACP日本支部2018年次総会で黒川賞を頂き、大変光栄です。2018年度から”Best Abstract”は「黒川賞」に変更となり、最初に黒川賞を受賞できたことを大変名誉に思います。
研究の経緯
今回研究を行うきっかけとなったのは、眼底鏡診察を、自信を持って実施できる内科医は少ないと感じたことである。自分の周りも見回しても、我が師匠である生坂先生を除いて、眼底鏡を実際の診療で積極的に活用している医師はほとんどいなかった。共同研究者である菊川誠先生（九州大学大学院医学研究院 医学教育学講座）が発表されているが、初期研修医、内科専攻医、内科指導医のいずれも、約90％は眼底鏡を利用していないまたは2、3ヵ月に1回の使用頻度とされる。その理由としては、眼底鏡診察が苦手というのがほとんどの理由であった。
自分が眼底診察を教えてみると、なぜ観察が難しいのかに気付いた。それは、学習者と指導者が同じ画面を共有できないことにある。学習者からすれば、今確認しているものが、本当に正しいのかについてフィードバックを受けられず、確証が得られない。また指導医も、なぜ確認できていないのかを指摘するのは至極難しい。そんなジレンマを感じていた。
研究の転機
研究の転機となったのは2017年3月のスタンフォード大学医学部内科への視察であった。そこで、エロル・オズダルガ医師と出会い、眼底鏡の教育にiExaminer systemを用いた教育手法について学んだ。彼とは、この教育手法を日本に持ち帰り実践したいこと、また教育効果についてリサーチしたいことを語りあい、快く承認してくれた。
千葉大学医学部クリニカル・クラークシップへの導入
まず、このiExaminer systemを用いて実際の教育を行ったところ、学生からの満足度は高く、今後の臨床実習でも積極的に使っていきたいという声が相次いだ。次に、本当にスキルが向上するかを確認するために、本学のシミュレーションセンターである、クリニカル・スキルズ・センターへ眼底鏡シミュレータの協力を仰いだ。教育の前後で評価することで、教育効果を明確にするためだ。題材は何にするか、問題数はいくつにするかなど、共同研究者とのやり取りは白熱した。特に今回の研究は「観察力」の評価だったので、そこに「解釈力」の評価が混入しないように腐心した。また、教員による教育効果は二重盲検化がデザイン的に困難であったため、最小限にするための工夫を行った。
今後の期待
今回の研究では、iExaminer systemを用いることで、眼底鏡診察の診断精度が向上し、かつ観察時間が短縮されることが示された。iExaminer systemは約8000円のアダプターと、今日だれもが携帯しているiPhoneさえあれば、フリーで利用できるため明日からでも実施可能である。この教育手法が広まることで、自信を持って眼底を診察できる医師が１人でも増えることを祈ってやまない。
