Implications from Experiences by Korean, Japanese, and Taiwanese Medical Professionals

| Purpose of the project | 国/地域において、耐え難い苦しみを経験して死を望む患者の尊厳をどう守るべきか、課題は未解決のままです。各国の専門家から実践知と理論知を学ぶことにより、最善の緩和ケアを提供するための方策を探索します。 When a seriously ill patient wishes to die， how should healthcare providers accept their wish and how should we care for them in accordance with relevant legal guidelines? The issue of how to protect the dignity of patients who experience intolerable suffering and wish to die remains unresolved in regions where active euthanasia and physician-assisted suicide are not permitted. By learning practical and theoretical knowledge from experts， we hope to explore ways to provide the best palliative care. |
Palliative care for children with severe neurological impairment

| Purpose of the project | 小児緩和ケアの対象疾患は、がんだけでなく非がん疾患が多いことが特徴です。中でも神経疾患の患者の割合は高く、米国の小児緩和ケアチームの対象患者の約3割が先天奇形・染色体異常及び神経疾患のこどもたちと報告されています。日本でも5歳未満の死亡原因の1位は先天奇形、染色体異常であり、緩和ケアニーズは高いと考えられますが、わが国おいて小児神経疾患に対する緩和ケア介入はまだまだ進んでいないのが現状です。 小児神経疾患は予後が不確実であり、エビデンスが限られていることから、治療方針の決定において様々な葛藤が生じやすくなります。また、多くのこどもたちが様々な症状を抱えているとされていますが、その評価が難しく介入につながらない実態もあります。 今回、Boston小児病院で長く小児神経疾患の緩和ケアに取り組まれ、この領域の第一人者であるJulie Hauer先生をお招きし、小児神経疾患の緩和ケアの実践についてお話しいただくことになりました。米国での実践から私たちができることについて考える機会になればと思います。 |

| Purpose of the project | 本講演では、International Journal of Behavioral Medicine誌の編集長・国際行動医学会の次期理事長で、サイコオンコロジーの専門家である、Hoyt教授に、男性の若年成人のがん患者に対する、心理社会的介入に関するご講演をいただきます。本講演により、若年成人のがん患者さんに特有の問題点と、それに対する治療介入方法の知見を学習する機会を提供したいと考えています。さらに、このご講演を契機に、日本における臨床や研究が発展することを期待しています。 |
An Integrative Medicine Approach to Pain Management

| Purpose of the project | 統合医療では、鍼治療やマッサージ、音楽療法、ヨガなどの運動療法をはじめ、幅広いアプローチが症状緩和に用いられています。海外では米国臨床腫瘍学会(American Society of Clinical Oncology: ASCO)や統合腫瘍学会(Society for Integrative Oncology: SIO)等からエビデンスに基づいた症状緩和における統合医療の推奨が出されています。アメリカのがんセンターでどのように疼痛のある患者に対して統合医療的なアプローチが利用されているのか知ることで、本邦でも新たな取り組みにつながるかもしれません。本セッションでは、がん患者における統合医療をご専門にされている緩和ケア医で、MDアンダーソンがんセンターで統合医療を実装されているGabriel Lopez先生が来日され、痛みに対する統合医療の観点からのアプローチについて、学術的、臨床的、そして教育的なご講演くださいます。このまたとない機会に、統合医療のエビデンスや臨床応用について多職種の皆様と共に学びを深められれば幸いです。 A wide range of approaches are used in integrative medicine for symptom relief， including acupuncture， massage， music therapy， yoga， and other forms of exercise therapy. Overseas， the American Society of Clinical Oncology (ASCO) and the Society for Integrative Oncology (SIO) have issued evidence-based recommendations for integrative medicine in symptom palliation. Knowing how integrative medicine approaches are used in cancer centers in the U.S. to treat patients with pain may lead to new approaches in Japan. In this session， Dr. Gabriel Lopez， a palliative care physician specializing in integrative medicine for cancer patients and implementing integrative medicine at MD Anderson Cancer Center， will visit Japan to give an academic， clinical， and educational presentation on the integrative medicine approach to pain management. We hope to take this unique opportunity to learn more about the evidence and clinical applications of integrative medicine with interdisciplinary health care professionals. |
