I am Masahiro Nonaka, and I have been appointed as the Chairman of the Japanese Society of Pediatric Neurosurgery starting from June 2023. Following in the footsteps of my predecessor, Dr. Isao Date, I am committed to striving for the smooth operation and development of our society with the support and guidance of its members.

Our society was established in 1972 as a research association and transitioned to a board of director system in 2012. Dr. Hajime Arai served as the inaugural Chairman, followed by Dr. Hiroaki Sakamoto in 2015, and Dr. Isao Date in 2019. Now, in 2023, I have been honored to assume the role of the fourth Chairman. During this period, we introduced the pediatric neurosurgery certification system in 2016 and officially became a general incorporated association in 2017.

Japan is facing a demographic challenge of population decline, and pediatric neurosurgery, as a subcommittee of the Japanese Neurosurgery Association, is expected to experience this decline at the fastest and most significant rate. When our society was founded, the birthrate in Japan exceeded 2 million, but by 2022, it had sharply decreased to 770,000. Examining the total fertility rate by prefecture, Okinawa, Shimane, and Miyazaki prefectures are ranked as the top three, all with fertility rates exceeding 1.6. In contrast, Tokyo, at the bottom of the list, has a rate of 1.13. This indicates that while fertility rates are declining in urban areas, they remain relatively high in rural regions, ensuring a reasonable number of children are being born. Therefore, I believe that maintaining and enhancing the level of pediatric care in rural areas will be a significant challenge in the future.

Patients with conditions that can be managed on a non-urgent basis but are complex to treat can be referred to experienced medical facilities in large cities. However, patients requiring urgent treatment, such as those with traumatic injuries or acute hydrocephalus, should have access to standardized medical care anywhere in Japan. To achieve this, we are working to establish treatment guidelines and to ensure the presence of as many board-certified pediatric neurosurgeons as possible in major cities across Japan.

Additionally, we consider academic activities in pediatric neurosurgery to be a major challenge. As mentioned earlier, the declining birthrate will limit the number of cases that can be experienced at a single institution. Currently, numerous English-language papers on pediatric neurosurgery are published in Japan, but most of them are case reports. Unfortunately, the number of English-language journals that publish case reports is decreasing. In order to continue publishing high-quality English-language papers in the future, we believe that multicenter collaborative research should be more actively conducted. Currently, the Japan Neurosurgical Society is leading a shunt registry in which we are actively registering cases, and we intend to leverage this experience to support and promote multicenter collaborative research on other clinical issues in the future.
