Keiichi Ichimura, MD, PhD
Department of Otolaryngology-Head and Neck Surgery
Jichi Medical University School of Medicine
I am privileged to say it is my great honor to be elected as a President of 46th Congress of Japan Rhinologic Society. Our department was established in 1974 with so famous Prof. Kirikae as the first chairman. Since then, this is the second time for us to hold national academic congress since Prof. Morita held Congress of the Society for Japan Head and Neck Cancer in 1990. Thus, we are all strangers to holding congress. We, however, will show our presence in a cordial and friendly atmosphere.
On the first day (Sept. 27) we will hold 43rd Meeting for Basic Study in Rhinology (MBSR), 36th Meeting for Clinical Study in Rhinology (MCSR), and sponsored lecture in Hotel Higashi-Nihon in Utsunomiya. The topic of MBSR named as Movement of water at nasal mucosa will be chaired by Prof. Watanabe and Prof. Shimizu. Nasal mucosa has an important role in the defense mechanism of the lower airway through warming and humidification. From the clinical viewpoint, dry nose has now been shared the spotlight with dry eye and dry mouth. This phenomenon is closely related with the symptom of old men's drip or postnasal drip in the elderly. Aquaporins are proteins constituting cellular water channels and are closely related with brain edema or cataract. They may play important roles in water exchange in the nose. In addition to this issue, ion transport in pathological conditions and vaporization from nasal mucosa may also be discussed. In MCSR Prof. Tomoda and Prof. Kikuchi will moderate the symposium titled Transnasal approach to the orbit. Endoscopic dacryocystorhinostomy (DCR) has become popular, but had not accepted as a topic in JRS meeting. Four experts, including ophthalmologist Dr. Kurihashi, meet together this time to show their techniques and wide knowledge. The sponsored lecture is designed as video presentation on ESS, especially frontal sinus surgery by Prof. Leopold in University of Nebraska Medical Center. Prof. Majima will host this lecture.
On the second and third day congress will be held in Tochigi Prefecture Cultural Center. I, as a Congress President, will give a lecture titled Management of epistaxis in patients with Osler's disease. In this lecture I will stress terrible condition of the patients and present how to manage them through my 20years'experience. We will provide 2 special lectures and an invited lecture. The first special lecture is done by Prof. Lund in Royal National Throat and Ear Hospital titled Evidence-based rhinology. She is my favorite person. She is not only a beautiful doctor but also has ability to present well-organized lectures and articles with clear intellect and articulate pronunciation in plain English. Prof. Suzaki, President of JRS, will chair the lecture. The second special lecture by Prof. Leopold is on dysosmia. He is the first to cure dysosmic patient with his fine technique to peel off olfactory mucosa. Prof. Furukawa will preside at this lecture and glean the details. Prof. Yoon is a top researcher in Korean Rhinology. As a delegate from The Korean Rhinologic Society, he will come to present a lecture titled Regulation of mucin gene expression in human nasal mucosa. A symposium will be held on tissue repair in the nasal mucosa moderated by Prof. Kurono and Prof. Nibu. Injuries and programmed apoptosis are accompanied by tissue repair. Both respiratory and olfactory mucosa will show a similar but distinct restoration mechanism. It is exciting to imagine cross-talk of involved cells. We have 2 panel discussions. The first will be hosted by Prof. Kishimoto on anterior and anterolateral approach to the skull base. This topic is rather unfamiliar for practitioners and may be suitable for the Congress of skull base surgery or head and neck surgery. But the knowledge and technique on this issue is quite applicable to clinical practice in rhinology and provide a clue for solving daily problems. The second one is titled as Traditional or oriental medicine-oriented approach in rhinology is chaired by Prof. Ogino. Some say oriental medicine has no evidence. If so, it had perished from the earth. Clinical trials for more than thousands years has proved the efficacy of the medicine. Clinical environmental arrangement to feel free from prescribing Chinese medicine is desirable. Instructional seminars are planned in a hands-on form. One is hosted by Prof. Iino and Dr. Sano and on management of pediatric patients with nasal diseases and another is regarding clinics on external nose hosted by Prof. Kase and Prof. Kubo. We have 5 luncheon seminars and Korea-Japan interrelation session.
We are very happy if you come and join in the coming Congress of JRS. Any topic relating with Rhinology is welcome for oral presentation.
Utsunomiya city is about 100 km north to Tokyo, and it takes less than an hour by Shinkansen. Utsunomiya City boasts the largest gyoza consumption in the country. So, in 1993, the municipal government started to promote Utsunomiya as the city of gyoza. About 800,000 tourists now visit the city annually to enjoy the specialty. Recently, government is also giving publicity to Jazz and cocktail as a city symbol. You may ask why. The reasons are that Sadao Watanabe, famous jazz sax player, was born in this city, and a slew of bartenders who have won the first prizes in the world as well as in Japan gather around Utsunomiya. So, enjoying jazz and cocktails in Utsunomiya is a must. Nearby Utsunomiya, Nikko and Nasu is famous scenic beauties. "Shrines and Temples of Nikko" were registered as a World Heritage site at the 23rd session of the World Heritage Committee held at Marrakesh in Morocco in December 1999. The site consists of two Shinto shrines and one Buddhist temple (Futarasan Shrine, Toshogu Shrine, Rinnoji Temple) and their surrounding remains, and includes 103 buildings of which nine are classified as National Treasures and 94 as Important Cultural Properties. The picture on the top page of this homepage is Yomeimon, probably the most famous work of architecture in Nikko erected in 1636. In addition, you can enjoy many cozy little neighborhood hideaway hot-spas around here and it is also good for you to go a little further to Mashiko, famous place for Mashiko ware, or to Motegi for joining motor sports amusements in Twin Ring Motegi.
