• 1962年Arnold P. Friedman（1909〜1990, ニューヨークの内科医）らが中心となった米国神経学会・頭痛分類特別委員会の頭痛分類（Ad Hoc Committee on Classification of Headache）では15項目の分類を発表し、これが世界的に長く使われてきた。
  　この分類は、頭痛を「機能的な頭痛」と「基質的変化に基づく頭痛（症候性頭痛）」に分けていた。さらに、機能的頭痛を「血管性頭痛」と「緊張性頭痛」に分け、片頭痛や群発頭痛などを血管性頭痛としてまとめていた。

• 国際頭痛学会の頭痛分類と診断基準
• 初版は 1988 年に刊行され、これによって頭痛診断が標準化され、科学的な研究成果や治療経験を各国の研究者が共有し、比較検討することが可能となったため、トリプタンをはじめとする頭痛治療薬の開発や頭痛研究に大きく貢献した
• 2004年に第2版（ICHD―II）が、同年にはその日本語版が日本頭痛学会の新国際頭痛分類普及委員会の翻訳によって刊行され、本邦でも広く用いられてきた。
• 約 10 年ぶりに大改訂が行われ，2013年7月に第3版beta版（ICHD―3β）が公開された。

• Ad Hoc Committeeの分類↑では、片頭痛を血管性頭痛に分類するのは問題があるとする立場が生じ、Jes Olesenらを中心に新しい頭痛の分類が試みられ、1988年、国際頭痛学会 International Society of Headacheが中心となり、新しい分類(ICDH)を発表し、さらに2004年に改訂 (ICHD-II)された(分類2)。
  　この新しい新分類は13項目から成る。「機能性頭痛である片頭痛、緊張性頭痛、群発頭痛」と「基質的疾患に基づく他の頭痛（症候性頭痛）」に分けられていて、この点では、Ad Hoc Committeeの分類と同じである。
